Each donor can track every dollar contributed to the penny. You can login 24/7 to monitor your contributions. Contributors can target their health care donations to a specific individual, family, or community.
Are you interested in helping an individual, family, or community get the health care they need?
Recurring donations for patient care go directly to the participating provider office responsible for delivering the care. Unless specified by the donor, CHS Life will select the provider office and work with the provider to select an individual or family needing health care assistance. Once the provider signs the donation agreement a copy will be returned to the donor. Please click, print, and mail us the donation agreement located on this page.
