The CHS Life “Local Membership Plan”, allow you to purchase a membership to a specific provider or group of providers. Our local health plans are designed by our participating provider offices. These plans offer significant savings to those patients requiring treatment for chronic conditions and preventative care.

Your membership can be self funded, employer funded, charity funded, government funded or any combination of the funding options mentioned. 5.
