2. Family Membership (Two adults plus dependent children): 3. Member Choice Distribution: 1. Basic Individual Membership: Cost: $100 monthly. Membership includes 2 hospital stays annually, 1 primary care office visit annually, 1 dental cleaning annually, and 8 miscellaneous health care office visits to be used at member’s discretion. Membership includes 4 hospital stays annually, 2 primary care office visit annually, 2 dental cleaning annually, and 16 miscellaneous health care office visits to be used at member’s discretion.    All additional units of care $125 Cost: $200 monthly.   State Responsibilities: Participating Provider Costs:  



The day to day operational cost for an entire CHS Life health care system can be paid for by ether the participating providers or the government designing the program. The goal is to keep the total cost of managing the both global and local membership at each provider office and hospital to under 3%.     The initial cost to cover customer service, technical support, and training for each provider office is $45 per month. Each office is completely automated to process global and local memberships.   Initially it will cost $3,000 a month to cover the customer service, technical support, and training for each participating hospital. Each hospital is completely automated to process global and local memberships. The States responsibility is one of design which includes determining price and number of units of care earned. The State also needs to provide auditing and accounting services for the entire system.
